    Provides services and activities in the home, supporting people with disabilities to live independently and in the community. People supported through the program are not living within funded shared accommodation services. This may include shopping, budgeting or learning a new skill to enable people to continue living independently and to easily participate within their community.

        Families registered with Department of Human Services and must have Case Manager assigned
        Families living within DHHS Southern Metropolitan Region
